Opening Doors – Rapid Re-Housing
About this Program
Every person deserves a safe place to live. Our housing stabilization program helps individuals and families experiencing homelessness transition into permanent housing through time-limited rental assistance and wraparound case management that supports long-term success.
Program Overview
This program is part of a coordinated, community-wide effort to prioritize the most vulnerable residents for housing support.
Coordinated Entry Required: All referrals must go through the Coordinated Entry system, which includes the VI-SPDAT (Vulnerability Index – Service Prioritization Decision Assistance Tool) to assess and prioritize need.
Referral Contact – HEART Team Call 864-591-4417 or email heart@aptchspartanburg.org.
Eligibility Referrals are accepted for individuals and families currently experiencing homelessness and identified as highly vulnerable through Coordinated Entry.
