About this Program

The Homeless Engagement and Response Team (HEART) is a direct outreach initiative led by A Place to Call Home in partnership with the City of Spartanburg. Focused on building trust and lowering barriers, HEART meets people experiencing homelessness where they are—offering personalized support instead of one-size-fits-all solutions.

Our Goals
Reduce homelessness in Spartanburg County by 10%
Refer at least 75% of individuals to the right local resources
Strengthen relationships with downtown businesses and the wider community through education and outreach

How HEART Works

Daily Outreach: Community Resource Coordinators (CRCs) provide consistent street-level engagement, rotating on-call coverage throughout the week.

Individualized Support: Every plan is tailored to each person’s unique needs, whether housing, health care, or employment.

Referrals & Prevention: Calls and emails are screened daily; families and individuals at risk are quickly connected to resources and case management.

Business & Community Engagement: Weekly downtown walks and outreach dinners build trust with both unsheltered individuals and local businesses.

Collaboration: HEART partners closely with shelters, law enforcement, healthcare providers, and housing agencies to create a coordinated system of care.
